Client and Therapist Rights & Responsibilities


  • EXISTING & NEW MEDICAL CONDITIONS

    • It is the client’s responsibility to inform Massage Innovations of any medical treatment or new medications currently being taken, and to provide written permission from the Physician, Chiropractor, Physical Therapist, etc.
    The client must also keep the Massage Therapist informed of any changes in health conditions.
    • If you have any questions on the above information, please discuss with Massage Therapist at time of session.

    MUTUAL RESPECT

    • Our Massage Therapists are happy to adjust pressure, temperature, musical volume, change technique, if you request it.
    • We treat all client visits and conditions with respect and confidentiality.
    • Please avoid wearing strong perfumes or cologne to prevent any allergic responses.
    • Please communicate with the Massage Therapist, (We really don’t mind readers!)
    • All Therapies are professional, therapeutic massages, there will be NO sexual contact or conduct at any time between any client and therapist. Should a client behave in a manner that violates this professional boundary, the session will be ended promptly and the client will still be required to pay for the session in full.

    SCOPE OF PRACTICE

    • Massage Innovations Therapists are Illinois Licensed professionals and are held to the professional standards of the American Massage Therapy Association.
    • Massage Therapy is a profession in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well-being of the client.
    • Licensed Massage Therapists do not diagnose or prescribe for medical conditions. Our therapist may refer you to a medical doctor for diagnosis or treatment. Our services are not intended as an alternative for proper medical attention for specific conditions. We will, however, communicate with you about what is in our scope of practice.

    INFORMED CONSENT

    • Prior to each Massage session, the treatment plan will be discussed between the Massage Therapist and Client. At your first visit, you will be asked to sign this policy and procedure/treatment consent form stating that you have read, understand and agree to comply with the professional massage therapy procedures. Clients who we have not seen for at least a year may also be asked to fill out this form.

    LATE ARRIVAL POLICY

    • All treatment sessions have a time scheduled. Late arrivals may not receive extension of scheduled appointments. In special cases, and when our schedule will allow, we may be able to accommodate a partial or full appointment. This will be at our discretion and only with proper advanced notification of your late arrival. The original appointment fee will be charged.

    CANCELATION AND NO-SHOW POLICY AND FEES

    • Your Session times are reserved exclusively for you. We value your business and ask that we can work together to keep the office scheduling policies. Should you need to cancel or reschedule, please notify us at least 24 hours in advance.
    • Any cancellations with less than 24 hours’ notice are subject to a cancellation fee of $20. When you miss an appointment with us, our therapists not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time.
    • At Massage Innovations by TLC, we understand that unanticipated events occur in everyone’s life. Unforeseen events such as car problems, business meetings, and children’s illnesses are just a few reasons why one might consider canceling a massage appointment. However, we ask that you call if you cannot keep your appointment.

    INCLEMENT WEATHER POLICY

    • If an event or session is canceled because of inclement weather, no fee will be charged to the client as long as the event or session is rescheduled for a future date. Massage Innovations reserves the right to cancel an event or session in the case of inclement weather if it jeopardizes the safety of our therapists and clients.
